(a) the details of beneficiaries of insurance cover provided by Insurance Companies
during each of the last three years category-wise including vehicle owners and
salaried drivers;
(b) the details of guidelines being followed in according such benefits;
(c) whether the implementation of such guidelines is monitored; and
(d) if so, the details of misapplication of provisions noticed during the same period and
steps taken therefrom?
