(a) whether Infant Mortality Rate and Child (0-4 years) Mortality Rate (both rural and urban) in Orissa are much more than the national average;
(b) if so, the details thereof during the last three years;
(c) the preventive measures being taken to tackle such menace; and
(d) the present status of National Immunisation Programme in Orissa?

Child (0-4 years) Mortality Rate in Orissa and India for the last three years are given below:-
