Question : Quality Assurance Committee

Will the Minister of HEALTH AND FAMILY WELFARE be pleased to state:

(a) whether the Quality Assurance Committee (QAC) and the District Quality Assurance Committee (DQAC) has been set up in every State and District as per the directions given by the Honourable Supreme Court in Ramakant Rai and Devika Biswas case;

(b) if so, the details and necessary particulars of each member of the QAC and DQAC;

(c) whether the incident audit is conducted by the Government or any State/ UT Government with respect to sterilization deaths and if so, the details in this regard including the steps taken by the Government against the doctors or people involved in the sterilization procedure for the death of a patient or any failure, other complication connected with the sterilization;

(d) whether the Annual Report prepared by the QAC indicates the details of all inquiries held and remedial steps taken as directed by Honourable Supreme Court judgement and if so, the details in this regard;

(e) whether the QAC has published an Annual Report containing not only the statistical information as earlier directed, but also non-statistical information in the form of a report card indicating the meetings held, decisions taken, work done and the achievements of the year etc.; and

(f) whether the first such Annual Report covering the calendar year 2016 is published on the websites mentioned above on or before 31st March, 2017 and if so, the details therefor?

(a) & (b): Yes. Quality Assurance Committees (QAC) have been set-up in every state and district as per the directions given by the Hon’ble Supreme Court.

The quality assurance committee comprises of 15 members at the state level and 12 members at the district level. State wise details and necessary particulars are available at the National Health Mission website.


(c): The District Quality Assurance Committees (DQACs) conduct incident audit with respect to deaths, failure or complications following sterilization. The audit report elicits remedial actions initiated for correction as per recommendations of the committees.

(d) to (f): The Hon’ble Supreme Court directed Government of India and State/ UT Governments to publish Annual report containing not only the statistical information, but also non-statistical information on meetings held, decisions taken, work done and the achievements of the year etc. The Ministry of Health and Family Welfare taking cognizance of the order has sent letters to all State Governments and has conducted dissemination workshops in states to apprise them on the steps to be taken to implement directions of the Hon’ble Supreme court. Complying to the same, the states have uploaded non-statistical information of the list and particulars of empaneled service providers on their State websites which are also linked to the National Health Mission portal.
