Answer given by the minister

THE MINISTER OF STATE IN THE MINISTRY OF HEALTH & FAMILY WELFARE (SMT. PANABAKA LAKSHMI)

(a) to (c) As per the provisions of the Pre-conception and Pre-natal Diagnostic Techniques Act 1994 and Rules 1996, it is mandatory by a registered clinic/centre to fill in data in one of the forms D to F in respect of every woman or man subjected to pre-natal diagnostic tests. Ultrasound clinics are required to fill Form F for each pregnant woman undergoing ultrasound scan. A copy of the filled in forms is required to be maintained at the clinic and a complete report is required to be sent to the Appropriate Authority by 5th of the following month for which report is to be sent.

A data entry and report generation software for Form F has been developed and sent to the selected States for pre-testing before launching the same country-wide.
