(a) the amount spent on each citizen under Health Protection Services Head;
(b) the yearly budget allocations being made for the purpose;
(c) the amount being provided to Madhya Pradesh every year under the said Head since, 1998;
(d) whether this amount has been utilised properly; and
(e) if not, the reasons therefor?

(a): There is no such head as Health Protection Services being operated by the Government. However, as per information furnished by the Central Statistical Organisation, per capita Government expenditure on health at current prices for the period 1996-97 was Rs.97.00.
